If you created a new form and sheet is not attached to the form, Boloforms may not work properly hence we ask users to create a sheet manually.

1. Go to responses tab and Click on sheet icon

1. Click on create a new spreadsheet and Click CREATE

   

2. Now reload the form and open Boloforms it should work as expected
